Saint Jude uses images, interviews, and videos of our children for a variety of internal and external communications. Our forms of internal and external communications include but are not limited to: print, such as newspapers, bulletins, and newsletters; photographs and digital images; film and videos; web posts, web pages, and image carousels.

Vestment Agreement

Nearly $5,000 has been invested by Saint Jude the Apostle parish in order to provide our Altar Servers with their own vestments. Being good stewards of this investment is the responsiblity of all involved in our Altar Server Guild. Therefore, Altar Servers and their parents are asked to consent to this agreeement.

Server Agreement*

I understand that I will receive Altar Server Vestments. I understand that it is my responsibility to properly care for these and any future vestments that I receive. Once I receive these vestments, I understand that I will need to arrive for my Altar Server assignments with these vestments and that they must be in a condition appropriate for serving (neat and clean.) I also understand that it is my responsibility to prevent damage beyond normal wear so that my parents do not have to repair or replace them.

Parent Agreement*

As a parent for the above Server, I am willing to help my child properly care for all vestments that we receive. I understand that as my child grows, we will be able to trade out vestments in good condition for larger sizes. I understand and agree that when my child is no longer a part of the Saint Jude Altar Server Guild all vestments must be returned to Saint Jude. I also understand that we will be responsible for the replacement of any vestment pieces that we are unable to return in good usable condition (no damage beyond normal wear.)
